Redeeming FlexPoints for Rewards
Available Airline Rewards, Gift Certificate Rewards, Merchandise Rewards and any other Program Rewards as well as the number of FlexPoints needed to obtain each reward are available to cardmembers at the FlexPerks Rewards Center or by phone at 888-229-8864. U.S. Bank and/or Maritz may change or terminate participating merchants, withdraw, limit, modify, or cancel any rewards, add an unlimited number of blackout dates, or increase the number of FlexPoints required for any reward at any time without advance notice, even though such changes may affect the accrual or value of FlexPoints or rewards.

FlexPerks Travel Rewards Visa Signature cardmembers may redeem FlexPoints for rewards only if the outstanding balance of their account is below the revolve limit. All FlexPerks Visa cardmembers may redeem FlexPoints for rewards if their account is open and in good standing. We reserve the right to verify and adjust FlexPoints at any time before or after posting any redemption. Once you or we close your account for any reason, all accumulated FlexPoints may be forfeited.

FlexPoints will expire five years from the end of the calendar quarter in which they were earned. FlexPoints are redeemed or expire in the order in which they were earned and awarded to the account. All FlexPoint redemptions are final.

For redemptions, information about the current Program Rewards Schedule, inquiries, and updates, cardmembers may visit the FlexPerks Rewards Center anytime or call 888-229-8864 between 8:00 am and 11:00 pm (CST) Monday through Friday and 8:00 am to 9:00 pm (CST) Saturday and Sunday.

Air Travel Rewards
All reservations must be booked online through the FlexPerks Rewards Center or by phone with Quality Reward Travel at 888-229-8864. FlexPoints may be used to purchase travel rewards. Airline ticket value, which includes taxes and destination charges, will be based on the current Program Rewards Schedule.

Internet Reservations
For online award travel ticket reservations, you may use the Travelocity engine at the FlexPerks Rewards Center to redeem FlexPoints earned in the Program for booking award tickets. FlexPoints for award travel tickets are considered fully redeemed when an airline issues a confirmation number for an e-ticket, paper ticket or voucher.

Award Travel Tickets can be issued in any individual's name, but FlexPoints used for the purchase must be redeemed by an authorized cardmember on the account. Tickets will be emailed to the trip contact provided when booking the ticket. Choice of carriers and traveling dates are at the cardmember's discretion and subject to availability. Note: Reservations on carriers with records of unreliable service, or other indications of unreliability, may not be available.

If an airline does not offer e-ticketing or if you request a paper ticket, there will be a minimum shipping fee assessed of $29.95 per transaction. U.S. Bank, Maritz Loyalty Marketing, Travelocity and Quality Reward Travel shall not be liable for any bodily and/or property damage that may result from your participation in the Program, or the performance by the airlines of the ticketed transportation; do not guarantee the availability of any flight on any airline or seats on any such flights; do not endorse, guarantee or warrant the goods and services offered by any airline. Phone Reservations
For redemptions using Quality Reward Travel: You may redeem FlexPoints earned in the Program only for a round-trip, scheduled ticket on any ARC (Airline Reporting Corporation) approved carrier as designated by Quality Reward Travel. Airline tickets will be non-refundable and non-transferable. Charter flights and consolidator fares are not eligible. Round-trip tickets must originate in the United States. To book an airline reservation, please call 888-229-8864 between 8:00 am and 11:00 pm (CST) Monday through Friday and 8:00 am to 9:00 pm (CST) Saturday and Sunday.

FlexPoints for award travel tickets are considered fully redeemed when an airline issues a confirmation number for an e-ticket, paper ticket, or voucher. Lost, stolen, or otherwise destroyed tickets or documents are not reusable or replaceable. A minimum of $135 per ticket fee will apply for changing/cancelling/reissuing a domestic travel reservation. A minimum $235 per ticket fee will apply for changing/cancelling/reissuing an international travel reservation.

All tickets will be issued as e-tickets unless e-tickets are not available. E-ticket confirmation can be accessed via the Internet. If the Internet is not available, e-ticket confirmation will be sent via fax or regular mail. If paper tickets are requested when e-tickets are available, a minimum $50 per ticket service fee will apply. A minimum $15 shipping/handling fee will apply for all paper tickets. Shipping/handling for tickets mailed outside of the 48 United States may be charged at a higher fee. To pay any additional fees, a credit card payment will be required at the time of booking.

Award Travel Tickets can be issued in any individual's name, but FlexPoints used for the purchase must be redeemed by an authorized cardmember on the account. Tickets will be available electronically, unless tickets are mailed, in which case they will be sent to the cardmember's billing address. Choice of carriers and traveling dates are at the cardmember's discretion, subject to availability. Companion tickets not redeemed with FlexPoints will be subject to all Quality Reward Travel issuance fees and restrictions. Cardmember must confirm flight reservations 24 hours prior to departure. Ticketholder must have proper identification for airport check-in. Tickets must be issued at the same time of booking. Tickets can not be redeemed for cash and have no cash value.

Hotel, Car Rental and Cruise Rewards/Gift Cards
You may redeem FlexPoints earned in the Program for gift certificates or gift cards. Gift certificates and gift cards are non-reloadable. Mechanical reproductions, copies, or facsimiles of gift certificates or gift cards will not be accepted. Additional terms and conditions appearing on the gift certificate/card shall apply to the use of the gift certificate or gift card.

FlexPoints for gift certificate or gift card rewards are redeemed when gift certificates or gift cards are requested by a cardmember. At the time of the gift certificate or gift card redemption, Maritz Loyalty Marketing will deduct the number of FlexPoints needed for that item from your FlexPoint balance. Maritz Loyalty Marketing will use its best reasonable efforts to issue a gift certificate or gift card within 7 to 10 business days of your request. Lost, stolen, destroyed or expired gift certificates or gift cards may not be reissued or replaced. Maritz Loyalty Marketing is not responsible for rewards or correspondence lost or delayed due to U.S. Mail delivery.

Gift certificate or gift card rewards will be mailed only to the cardmember's billing address. Gift certificates or gift cards will not be personalized and are transferable and may be redeemed by any person. Gift certificates or gift cards must be presented to participating merchants for use. Gift certificates or gift cards are valid when issued. We have no control over merchant acceptance of a gift certificate or gift card that is issued as a reward. Gift certificates or gift cards are not exchangeable, refundable, or redeemable for cash and cannot be replaced if lost or stolen.

Merchandise Rewards
You may redeem FlexPoints earned in the Program for merchandise available through participating merchants under the then-current Program Rewards offering located at the FlexPerks Rewards Center. You may not use cash or anything other than FlexPoints earned with your account to acquire merchandise through the Program. All items are subject to availability; most orders will ship within 4 to 6 weeks from the date of redemption. All substitution offers at the time of redemption will be of equal or greater value.

FlexPoints for merchandise rewards are redeemed when merchandise is requested by cardmembers. All FlexPoint deductions will be made at the time of redemption. Returns on merchandise rewards are only accepted for merchandise that is damaged, defective or mis-shipped. Returns will be honored for equal exchanges. Merchandise returns should be returned to the merchant or other issuer(s) of the merchandise or as otherwise directed. U.S. Bank will not be responsible for handling returned merchandise. No FlexPoint credits will be accepted or issued. The manufacturer's warranty, if any, applies to all items offered. Merchandise may only be shipped to a street address within the United States; however, APO, FPO, and P.O. Boxes are not included. Due to weight restrictions some items may not be available to be shipped to Alaska and Hawaii. Only standard shipping is available for merchandise rewards. No express delivery.

Statement Credit
Statement credit is issued via a credit to your account by accessing the FlexPerks Rewards Center when logged on to usbank.com. A credit will be applied to your account within one or two billing cycles.

Participation in the U.S. Bank FlexPerks Visa Program (the "Program") is restricted to U.S. residents who are 18 years of age or older ("cardmember", "you ", and "your") and in whose name U.S. Bank National Association ND ("U.S. Bank", "we", "us" and "our") maintains a FlexPerks Visa card account (an "account").
