By signing the account application, you agree to be bound by the terms and conditions set forth in the agreement.

• To apply by phone, call Investments Connect at 800-888-4700, Monday – Friday, 7:30 a.m. to 7:00 p.m. CT. They can also assist you in transferring an existing investment account to U.S. Bancorp Investments, Inc.
• To apply by mail, download and print either a Self-Directed Brokerage Application (PDF) or Discount IRA Application (PDF), complete the form, sign it and mail it to:
