You earn triple cash back - a full 3% on cellular, gas and purchases made at office supply stores and 1%
on all other purchases made with your U.S. Bank FlexPerks Business Cash Rewards Visa Card. You'll
also get an annual bonus equal to 25% of your prior year's cash rewards, calculated annually on
your account anniversary date. Employee cards are free and earn cash rewards too. Redeem your

| The Fine Print |

- Rewards are earned on net purchases only. Cash rewards will not be earned for unauthorized Account transactions, Balance Transfers, Cash Advances, Cash Equivalent Advances, Overdraft Protection Advances, Convenience Checks, Finance Charges or Fees.
- Rewards earned on open accounts in good standing only. If your account is closed for any reason, any reward remaining on the account will be forfeited.
- There is no cap on the amount of cash rewards you can earn and they are non-expiring.
- The cash rewards are calculated monthly and a rolling total appears on your monthly statement. Log in to U.S. Bank Internet Banking and select Rewards Center or call 866-485-4545 to redeem your earned cash in $25 increments, with a minimum redemption of $25.
- Earned reward amounts will be printed on monthly statements.
- You can choose to redeem your cash rewards as a deposit into your U.S. Bank deposit account (checking, savings or money market), a U.S. Bank Rewards Card or a statement credit.
- Full terms of the FlexPerks Business Cash Rewards Visa program will be sent when you become a card member.
